    <Abstract>Background/Aims: Currently، liver disease is one of the main causes of mortality in adults. liver transplantation(LT) is the only way to treat patients with advanced liver failure and this treatment has been successful in developing countries. Several factors may affect to the long-term survival of liver transplant patients. The aim of this study was to determine the effective factors on survival of liver transplant patients in Shiraz Namazi Hospital between 2001 and 2018. Materials and Methods: In this retrospective cohort study, 3712 adult patients with advanced liver failure were studied. Demographic and clinical characteristics were extracted from the patients' records. Cox models were used to determine the effective factors on survival. Data were analyzed using SPSS23 software. Results: In this retrospective cohort study, 3712 adult liver transplant patients were evaluated and 742 patients (20%) died. 2348 (63.3%) patients were male. The mean (SD) age of patients was 42.3 (13.2) years (range: 19 to 74 years). In the multivariate model, the year of transplantation, recipient age, donor age, donor sex, MELD score, etiology of disease, the compatibility of the blood group and the number of transplants were effective on the survival of the patients. Conclusion: Liver transplantation is an acceptable and safe treatment option for adult transplant candidates. donor Age over 40 years, recipient age, re-transplantation, blood group, and MELD score above 20 were associated with reduced survival of transplant patients, and transplants performed after 2010 and female donors were associated with long-term survival after liver transplantation.</Abstract>
